By now you should have found a Verification for Membership Information Form arrive in your mail. This form was included in your annual Membership Dues Renewal Letter sent to your employer for payment.


**It is important that you complete this form in order for us to update the information in our database to ensure that you are receiving all correspondence and workshop information.**

Each member is receiving a Verification of Membership Information Form. Please review the form for accuracy, make any necessary edits and return to MWWCA, 60 Community Drive, and Augusta, ME 04330. This form can also be faxed to us at 207-626-5947.

A copy of the annual Application for New Members is accessible on our web site. Copies may be made of this form or you can access it here.

Please note: If a member has left employment and you would like the new membership to be for a new employee, a new member application must be filled out and sent in with the dues payment. Because all memberships are individual memberships, membership cannot be transferred to another employee, they must be processed as a new member.
